A co-axial modification of the basic T-piece system allows scavenging of waste anesthetic gases. Bain Circuit consists co-axial modification of basic T-piece system. The tube carrying fresh gas travels inside outer reservoir tube for endotrachealing tube connector. Patients inspire fresh gas from the outer reservoir tube and expires into reservoir tube.

Construction

  • A fresh gas carrying tube travels inside an outer reservoir tube, right to the endotracheal tube connector.

Function

  • The Bain circuit functions just like the T-piece, except that, fresh gas supplying tube is based in the reservoir tube.
  • Inspiration -From the outer reservoir tube, the patient inspires fresh gas.
  • Expiration - The patient expires gas into the reservoir tube. Fresh gas continues to flow in the system, it can be wasted as it is contaminated by expired gas.
  • Expiratory pause - The expires gas is washed out of the reservoir tube by the fresh gas from the inner tube. Fresh gas fills in for the next inspiration.


Operational requirements

  • Same considerations apply as for the T-piece.
  • The efficient Bain eliminates exhaled gas, since the fresh gas is directed down the endotracheal tube, reducing the dead-space.

Modifications

  • Just like in T-piece, a bag can be added to the tail of the reservoir tube.
  • Alternatively, a pop-off valve can be used to attach circuit and a block assembly. It can be mounted to the common gas outlet of the anesthesia machine. This arrangement allows scavenging and intermittent positive pressure ventilation.
